Job: Geriatric Care Manager

Caring Cooperatives, a geriatric care management provider is looking to hire a per diem care manager for providing back-up and on-call coverage. While this position is primarily for on-call potential exists for the development of an individual case load.

Responsibilities include but may not be limited to:

Responding to emergency calls by current client base
Fielding In-take calls
Assessment and Comprehensive Care Planning
Home Safety Evaluations
Crisis Intervention, Management, and Prevention
Serve as family liaison
Coordinate referral and implementation of outside service providers

Qualifications:

Degree in social work, nursing, counseling or other related field.
Documented work experience and knowledge in geriatrics.
Working knowledge of community resources related to older adults.
Effective time management and organization skills.
