Intermountain Centers is a private, not-for-profit service organization, which provides residential and home based services to a variety of clinical populations. Intermountain programs are designed to develop interpersonal and social skills, self-sufficiency and self-management skills, academic and vocational skills, community awareness and responsibility and constructive future planning.
We are currently recruiting for In-Home Support Services Specialists, for our intensive services team, to provide behavior management, rehabilitation, support and training to clients and families in Pima County where one or more family members may be placed outside the home in foster, group or institutional care, or are returning to the home following an out-of home placement. Job Duties and Responsibilities: • Provides in-home treatment and support services to children and families in a community setting, utilizing therapeutic approaches compatible with ICHD treatment philosophy, with an emphasis on empirical behavior techniques. • Provides education and rehabilitative skills training; and formulates goal-oriented treatment/service plans to prevent recurrence of crisis and promote adaptive behaviors to maintain or improve functioning. • Participates in the development and implementation of the crisis/safety plan; Strengths, Needs and Cultural Discovery (SNCD) and treatment team recommendations for follow-up support and services, within required timeframes. • Assists children and families in obtaining community resources and formal and informal services, and in developing natural supports as needed; while utilizing best practices in the most appropriate setting. • Coordinates services with all service providers and stakeholders in a consistent, timely manner. • Participates in all significant meetings involving the Child and Family. E.g. IEP’s, court hearings, medication reviews, and foster care review boards. • Advocates for children and families; and develops and maintains supportive relationships with children, families, and significant support systems by meeting frequently, according to program requirements. • Arranges and/or provides transportation for children and family members for community services as specified by the program. • Ensures initial contact and face-to-face contact are made within program’s required timeframe of referral. • Ensures client files are maintained in accordance with ICHD and contractual standards. • Completes all required documentation for crisis/safety plans; SNCD’s; service plans; assessments; monthly reports; progress notes; and closures, within required timeframes. • Maintains a minimum of documented client contact hours per week; completes progress notes using the appropriate codes, and submits them on a weekly basis to the billing department. • Completes and submits current, verifiable, documentation for mileage logs and flex funds for services. • Maintains an approved schedule and acceptable level of attendance. KNOWLEDGE, SKILLS AND ABILITIES: • Demonstrates thorough knowledge of behavior management strategies Mental Ability • Ability to respond appropriately to difficult behaviors • Ability to respond appropriately and quickly to emergency situations • Ability to work a flexible schedule including on-call and some evenings and weekends Stress Level • Ability to handle a high level of stress Physical Ability: • Ability to lift up to 50 lbs. and push or pull up to 150 lbs. Skills / Requirements
Education and legal requirements:
• Bachelor’s degree related to behavioral health preferred; or • Any bachelor’s degree and 1 year of Full-time behavioral health work experience; or • Associate’s degree + 2yrs. of Full-time behavioral health work experience; or, • HS/GED +18 hours of higher education in behavioral health +2 yrs. FT behavioral health work experience; or • HS/GED + 4 yrs FT behavioral health work experience • 1 year of FT behavioral health work experience required. • Emergency Safety Response Certification (ICHD provides training-must pass physical and non-physical components of training course) • Minimum 21 years of age. • Possess a valid Arizona Driver’s License and 39-month driving record that meets insurance requirements • Legal record void of any felony • CPR and First Aide Certified (ICHD provides training) • All employees must be fingerprinted Important Notes
