Intermountain Centers is a private, not-for-profit service organization, which provides residential and home based services to a variety of clinical populations. Intermountain programs are designed to develop interpersonal and social skills, self-sufficiency and self-management skills, academic and vocational skills, community awareness and responsibility and constructive future planning.
We are currently recruiting for In-Home Services Intensive Case Managers. This position is responsible for the on-going assessment of needs and coordination of treatment services for program participants identified as needing a high level of service coordination. Typical caseload is 15 families. The Intensive Case Manager acts as the primary point of contact for all Child and Family Team(CFT) participants to facilitate meetings and follow-up recommendations, to ensure continuity of care and service delivery, and to educate team members about the CFT process. One position is currently available for the Tucson metropolitan area. PRINCIPLE DUTIES AND RESPONSIBILITIES: • Assesses the family’s strengths and needs through the creation of a Strengths, Needs and Cultural Discovery (SNCD), Service Plan and Safety Plan. • Develops a comprehensive CFT to include natural and community supports and professional involvement for children. • Develops and monitors treatment plans with program participants and participating agencies • Facilitates CFT meetings to educate partners in the levels of care, service availability (both formal and informal) and the CFT process. • Schedules and coordinates transition CFT meetings with other ICHD In-Home Services, or subcontracting, staff. • Provides referrals to outside providers and community resources in accordance with HIPPA policies; coordinate transition for services. • Performs on-going assessment of needs and service intensity for clients, using identified tools. • Attends and participates in team meetings, clinical staffings as neede,; hospital discharge planning meetings, court hearings, and other relevant meetings to meet the treatment needs of program participants • Assists with discharge planning for children leaving residential treatment or hospitals, and being referred to ICHD for out-patient services • Maintain client records in accordance with agency and funding source requirements • Provide complete documentation and reporting of client services, in accordance with agency and funding source requirements. • Maintains an approved schedule and acceptable level of attendance Skills / Requirements
Education, Experience and legal requirements
• Master’s degree or a bachelor’s degree in a field related to behavioral health; a bachelor’s degree and at least one year of full-time behavioral health work experience; or an associate’s degree and at least two years of full-time behavioral health work experience. • Minimum 21 years of age • Possess a valid Arizona Driver’s License and 39-monthdriving record that meets insurance requirements; • Legal record void of any felony; • All employees must be fingerprinted Important Notes
Bilingual candidates encouraged; a $1500 annual differential is currently paid to employees fluent in Spanish (upon successful completion of testing for verbal and written skills).
