Program Description
The Primavera Foundation’s Transitional Housing Program provides very affordable housing to single adults without children, adult couples without children, and families who meet income eligibility. All programs are drug and alcohol free and are designed to assist residents in their transition into independent living. Job Description Provide case management services for program participants at assigned site(s). Job Duties 1. Meet weekly or as needed with each program participant assigned to caseload. 2. Assess program participants and assist them in establishing goals to maximize independence. 3. Develop and maintain case plans and detailed case records. 4. Ensure that program participants have access to appropriate resources, including referrals to outside agencies and other appropriate resources. 5. Act as a liaison with probation officers, parole officers, other organizations, other Primavera case managers, and family members to coordinate services. 6. Coordinate, facilitate, and/or attend staffings relevant to the progress of participants. 7. Participate in planning and facilitating group activities as needed and assigned. 8. Maintain program participant data in paper case files and in Service Point HMIS database. 9. Perform a variety of clerical functions related to case management duties. 10. Attend meetings as required. 11. Perform other duties as assigned. 12. Evening hours may be required. Skills / Requirements
1. Bachelor’s degree or higher in Social Work, Counseling, Psychology or related field.
2. Ability to work effectively with diverse populations 3. Experience working with homeless and near-homeless populations. 4. Effective verbal and written communication skills. 5. Knowledge of community resources and local referral organizations, including 12-step and other recovery-oriented support groups. 6. Basic computer skills (Word, Excel, internet and e-mail at a minimum). 7. Knowledge of effective case management charting techniques. 8. Exceptional ability to establish strong working relationships with participants and colleagues, maintain professional boundaries, and demonstrate professional ethics. Important Notes
Preferred Education and Experience:
